A. NEW BUSINESS PERMIT
- Preminary Requirements
- DTI/SEC
- Barangay Certificate of the Business
- Occupancy Permit (4th floor-Engineering Office)
- Cedula of Owner
- Corporate Cedula (if Corporation)
- Authority to File
- Basic Requirements (for all establishments)
- BIR Tax identification Card (photo copy)
- EMB Clearance/Validation (photo copy)
- FIRE Clearance (photo copy)
- Pag-IBIG Clearance (original copy)
- PhilHealth Clearance (original copy)
- SSS Clearance (original copy)
- Barangay Clearance
NOTE: For Photo-Copies, present the original copy for authentication. In case the original copy is not available, an authenticated/certified copy by authorized official of the issuing agency/office may serve as an original copy.
Procedure:- Submit application and complete documentary requirement
- Pay corresponding fees to the City Treasurers Office
- Claim Mayor’s Permit and Business Plate
B. RENEWAL BUSINESS PERMIT
- Basic Requirements (for all establishments)
- BIR Tin Card, if not yet submitted (photo copy)
- Building Annual Clearance, if required (photo copy)
- EMB Clearance/CY Validation (photo copy)
- FIRE Clearance (photo copy)
- Pag-IBIG Clearance (original copy)
- PhilHealth Clearance (original copy)
- SSS Clearance (original copy)
- Barangay Clearance
- CEDULA (show original)

C. NOTICE OF RETIREMENT

Procedure:
- Submit duly filled-up Application for Represent of Business
- Claim Certificate of Retirement
D. TRANSFER OF OWNERSHIP OF BUSINESS
- Requirements
- Affidavit of Transfer
- DTI of the new owner
Procedure:
- Submit Affidavit of Transfer
- Pay corresponding fees to the City Treasurers Office
E. MOTORIZED TRICYCLE OPERATORS PERMIT
- Original Copy
- Franchise
- LTO-Official Receipt
- LTO-Certificate of Registration
- CEDULA
Procedure:
- Submit Application form with requirements
- Claim Mayor’s Permit/Sticker
