Option 1

The following documents be surrendered to CEZA upon resignation of a foreign employee:

§  Original CWV certificate or affidavit of loss

§  Original letter request for cancellation of CWV addressed to Mr. Jose Mari B. Ponce, Administrator and CEO of CEZA

Please note that CEZA will cancel said CWV from their database. CEZA will issue a cancellation order to be surrendered to the authorized representative of the company concerned and will report to the Bureau of Immigration (BI) said cancellations, unless a certified true copy of their downgraded CWV was submitted to CEZA within the week of receipt of the said order. Your office or the foreigner is advised to transact with the BI immediately for the downgrade of their CWV to a Section 9(a) visa, if applicable. Note that this is the current practice.

Timetable for this procedure is dependent on the availability of Mr. Jose Mari B. Ponce, Administrator and CEO of CEZA.
 

Option 2

The following documents be surrendered to CEZA upon resignation of a foreign employee as part of the downgrading of a CWV to a Section 9(a) visa procedure:

§  Original CWV certificate or affidavit of loss

§  Original letter request for cancellation of CWV addressed to Mr. Jose Mari B. Ponce, Administrator and CEO of CEZA

§  Original passport

Please note that this procedure will mean you or the foreigner will have to transact with the BI directly upon release of the CEZA Cancellation Order and the original passport on which CWV was duly stamped Cancelled and signed by Mr. Jose Mari B. Ponce, Administrator and CEO of CEZA.

Timetable for this procedure is dependent on the availability of Mr. Jose Mari B. Ponce, Administrator and CEO of CEZA.
 

Option 3

The following documents be surrendered to CEZA upon resignation of a foreign employee as part of the downgrading of a CWV to a Section 9(a) visa procedure (similar to Option 2):

§  Original CWV certificate or affidavit of loss

§  Original letter request for cancellation of CWV addressed to Mr. Jose Mari B. Ponce, Administrator and CEO of CEZA. Please do not forget to mention the effectivity of resignation of the foreign employee and it should not be more that one (1) month from the date the letter request was received by CEZA

§  Original passport

Please note that this procedure will mean the foreign employee is expected to leave the Philippines after the release of the CEZA Cancellation Order and the original passport on which CWV was duly stamped Cancelled and signed by Mr. Jose Mari B. Ponce, Administrator and CEO of CEZA but before the effectivity date of the cancellation of the CWV. Note further that the effectivity of the cancellation of the of the CWV will be the day after the effectivity of the resignation of the foreign employee that was stated in the letter request for cancellation
